Weeks 4–8: Downsize
Correctly Fitted Shorter Barbell

Clothing and Fabric Contact
Loose, soft, breathable fabric over healing nipple piercings reduces friction trauma. Avoid lace, rough textures, and tight-fitting clothing that presses the barbell against the piercing site. A clean breathable cotton layer during initial healing is the easiest protective measure.
Sleeping Position
Sleeping directly on a healing nipple piercing creates consistent compression. Back sleeping is best during the initial months. If side sleeping is unavoidable, a clean soft barrier between the piercing and the mattress reduces pressure.
Sports and Physical Activity
High-impact activity can cause the barbell to move and snag. Cover the piercing with a breathable bandage during sports and rinse with saline afterward. Contact sports involving chest-to-chest contact should be approached carefully during the first 3–4 months.

Will I still have sensation after a nipple piercing?
This varies between individuals. Some people report increased sensitivity, some notice minimal change, and some experience temporary reduced sensitivity during healing that resolves once fully healed. Correctly placed nipple piercings do not damage the nerve supply. Sensation changes are generally temporary and associated with the healing process rather than permanent nerve damage.
Can I wear a bra during healing?
Yes — a soft clean cotton bra or bralette actually protects healing nipple piercings by reducing direct fabric friction and providing a stable barrier. Avoid underwire bras that press the bar against the tissue, lace or rough textures, and very tight-fitting styles. Loose soft cotton is the most comfortable and protective option during healing.
What are signs of a healing problem?
Normal healing: tenderness, occasional clear or milky discharge, light crusting, fabric sensitivity. Potential problems: green or thick yellow discharge with odor, spreading redness and warmth beyond the immediate site, significant swelling after the first 3 weeks, the bar appearing to tilt or migrate, or jewelry ends appearing closer to the skin surface than when first pierced.
