What Is Threadless Jewelry?

Threadless jewelry — sometimes called press-fit jewelry — is a two-component system consisting of a labret post (the piece that goes through the piercing) and a decorative top (the piece you see on the outside). The top is held in place not by threads but by a bent pin that creates tension against the inside of the hollow post.

The image above shows these two components separated. On the left: the flat-backed post — a titanium tube with a flat disc on one end. On the right: the decorative top with its pin, slightly bent. When the pin is inserted into the post, that bend creates an outward spring pressure that keeps the top firmly seated without any screwing mechanism.

How the Bent Pin Works

1
The pin is slightly bent
The pin extending from the decorative top is slightly angled — not straight. This bend is what creates the holding force. The degree of bend determines how firmly the top is held.
2
Insertion into the post
The pin is pressed straight into the hollow end of the post. As it enters, the bend creates lateral spring pressure against the inner wall of the post tube.
3
Tension holds everything in place
The spring tension of that bent pin against the post wall is what keeps the top from falling out. It’s firm enough for everyday wear but releases with a gentle pinch-and-pull for jewelry changes.
4
To remove: pinch and pull
To remove a threadless top, you grasp the post with one hand (from behind the ear or the back of the piercing) and pull the top directly outward with the other. The tension releases cleanly with no unscrewing required.

Why It’s Better for Healing

Traditional threaded jewelry — where you screw a threaded ball or top onto a threaded post — has a specific problem: the exposed threads. During a piercing, jewelry passes through a fresh wound. External threads (threads on the outside of the post) catch on tissue. Even internal threads (threads inside a ball that screw onto a smooth post) create a recessed gap where debris can accumulate against healing tissue.

Threadless: Benefits
No threads to catch on tissue during installation
Smooth post surface — nothing for debris to catch on
Fast, single-motion removal for jewelry changes
Interchangeable tops — one post, infinite looks
No risk of over-tightening against healing tissue
Threaded: Drawbacks
External threads tear tissue on installation
Recessed thread gaps harbor bacteria
Screwing motion can irritate fistula during changes
Each top is locked to its matching post
Easy to over-tighten, compressing soft tissue

The Post: Understanding Flatbacks

The labret post used with threadless jewelry has a flat disc on the back end (the end that sits against skin inside the piercing) rather than a ball, bead, or butterfly back. This is a flatback design, and it’s the standard in professional piercing for the same reasons threadless tops are: no pressure points, no movement, flat against skin.

Post lengths matter enormously. A post that’s too short compresses the piercing; one that’s too long moves excessively and creates irritation. Your piercer will fit the correct length for your anatomy and for the swelling that occurs during initial healing. Most piercings are downsized 4–8 weeks after piercing to a shorter post once initial swelling has resolved.

Downsizing

Initial posts are intentionally longer to accommodate swelling. Wearing an initial-length post past the swelling stage allows the post to move freely in the piercing, which delays healing. Come back to the studio for a downsize at the 4–8 week mark — it’s one of the most important steps in a successful heal. Learn more in our downsizing guide.

Changing Threadless Tops Yourself

On a fully healed piercing, changing threadless tops is something many people learn to do at home. The key requirements are: the piercing must be fully healed, your hands must be clean, and the new top must also be implant-grade material. Never change jewelry on a piercing that isn’t completely healed without professional help.

  • Wash hands thoroughly with unscented soap and dry completely.
  • Hold the flat back disc firmly between two fingers behind the ear or behind the piercing.
  • Grasp the current top with your other hand and pull it straight outward — no twisting or rotating.
  • Align the pin of the new top with the hollow opening of the post.
  • Press straight in until you feel the tension catch. It should feel secure but not forced.
  • If a top feels loose, the pin may need a slight re-bend; bring it to a professional to adjust.

Frequently Asked Questions

A properly fitted threadless top with the correct pin bend is very secure for everyday wear, including sleeping and exercise. The most common reason tops come loose is that the pin bend has worn down slightly over time or was not bent to the correct angle originally. If a top feels loose, a piercer can re-bend the pin in seconds.
Threadless tops are largely interchangeable between posts of the same pin gauge and diameter, but some brand-specific tolerances exist. Tops from reputable manufacturers like Anatometal, Neometal, and BVLA generally work across brands with matching gauge specifications. When in doubt, bring the post and top to your piercer for a fit check.
Threadless flatback posts work in most labret-style piercings — lobes, cartilage, nostrils, lip, Monroe, philtrum, and many others. For piercings that require a barbell (like industrials, navels, or curved barbells), threaded ends are necessary. Your piercer will tell you which style is appropriate for your specific piercing.
Implant-grade titanium threadless jewelry is extremely durable. The posts and tops themselves can last years or decades with normal wear. The pin bend may occasionally need adjustment over time. Decorative tops with gemstones should be inspected periodically to ensure the stone setting remains secure.
Yes — implant-grade threadless jewelry costs significantly more than fashion earrings. The cost reflects certified materials, precision manufacturing, and surface finishing to implant standards. For healing piercings, this investment directly affects healing outcomes. For healed piercings, it means long-term wearability without irritation.
