The Daily Cleaning Routine

Wash hands first — every time
Before any contact with the piercing. Unscented soap, clean dry hands. This applies throughout the full 6–18 month healing period.

Spray with sterile saline wound wash
Sterile 0.9% sodium chloride, no additives. Spray directly onto the piercing from both sides. Let it saturate any crust present.

Rinse in the shower
Warm water flowing over the piercing after the saline spray helps clear softened crust. Do not use washcloths — fibers snag on jewelry.

Twice daily only
Morning and evening. Over-cleaning cartilage is a real problem — it removes moisture and lipids that healing tissue needs and causes dryness and irritation.
Never Use
Hydrogen peroxide, rubbing alcohol, Bactine, Neosporin, tea tree oil, or any product with fragrance or preservatives. Sterile saline only.
Cartilage-Specific Considerations
- Sleeping position. Side sleeping on a healing helix is the single most common cause of prolonged helix healing. Use a travel neck pillow or a piercing pillow that keeps pressure off the ear. Back sleeping is best.
- Headphones and earbuds. Over-ear headphones compress the helix directly. In-ear earbuds are less problematic but can disturb the piercing on insertion and removal. Be deliberate during the healing period.
- Hair catching on jewelry. Long hair can snag on helix jewelry, creating acute trauma. Keep hair tied back, especially during sleep and exercise.
- Hats and helmets. Anything that presses against the upper ear needs to be put on and removed carefully. Tight-fitting hats can create prolonged pressure on a healing helix.
- Do not rotate or move the jewelry. Cartilage is less forgiving than lobe tissue. Every rotation disrupts the forming fistula.
How Long Does It Take?
Helix piercings take 6–18 months to fully heal — significantly longer than lobe piercings. Surface healing may appear complete at 4–6 months but internal tissue is still forming. Do not change jewelry based on surface appearance alone.

Aftercare Questions
Yes — cartilage piercings produce crust for longer than lobes. As long as the crust is white or slightly yellowish with no odor, it’s part of normal healing. If it’s green, smells different, or is accompanied by significant redness and warmth, come in for an assessment.
A donut-shaped travel neck pillow or a purpose-made piercing pillow creates a space for your ear so it doesn’t contact the pillow surface. This is genuinely one of the most effective interventions for slow-healing helix piercings.
Minimise it. Short sessions are less problematic than hours of daily wear. If you must use them, apply and remove carefully and do a saline rinse afterward. In-ear earbuds on the opposite side are a practical alternative during the healing period.
