Stage-by-Stage Timeline

Weeks 1–3
Acute inflammation
Redness, swelling, tenderness, and clear discharge are normal. The conch sits in the inner ear bowl — initial swelling can make the ear feel full or tender when touched. Clean twice daily, leave alone otherwise.

Weeks 4–12
Early fistula formation
Inflammation decreasing. The fistula channel is beginning to form through the flat cartilage. No earbuds, no sleeping on the ear, no touching beyond cleaning.

Months 3–6
Fistula development
Calmer appearance. Reduced crust, decreasing sensitivity. The fistula is thickening but still fragile. Maintain aftercare and initial jewelry regardless of surface appearance.
4
Months 6–12
Maturation
Surface healing appears complete. Internal tissue still maturing. Professional jewelry assessment appropriate — if confirmed healed, upgrade jewelry is reasonable.
5
Months 12–18
Full internal healing
Fistula fully formed. The piercing is truly healed. At-home jewelry changes and larger hoop styles are now viable.
Timeline Questions
Surface appearance isn't a reliable guide for conch healing. We recommend professional assessment before any jewelry change — and particularly before switching from a stud to a hoop, which is a significant change in how the jewelry interacts with the tissue.
